I am interested in providing a Demonstration or Discussion at my Booth
Products and Services: List all products and services being offered. These will be the only elements allowed in your booth. All Vendors must label all services and products clearly and accurately. "Spirit of Harvest" Expo will not be responsible for any claims or expense arising from claims arising from the services and products.
Brief Bio for Program
Fees: Booth Rental approx. 10' by 6' (8' x 3' table and two chairs provided) $40. Additional Space for a Massage Table $20. Shared booth fee additional $10 if applicable. Payment by Square Invoice or Check/Money Order made out to Karen Rollins and mailed to 7398 Roshon Avenue Reynoldsburg, Ohio 43068. Acknowledgement - please initial your agreement to all conditions of "Spirit of Harvest" Expo.
